Planning Your Admission

Your doctor will provide admission papers prior to surgery. Complete the forms and forward to the Centre. Our nursing staff will contact you on the day prior to surgery with detailed information to ensure your admission is smooth.

Getting There

The facility is centrally located in Malvern Avenue Chatswood, immediately adjacent to Chatswood Chase. Parking is available either in the street or in Chatswood Chase.

Day Of Admission

Take particular note of fasting details and times provided by the centre. Ensure you have arranged a responsible adult to accompany you home and care for you during the night.

What To Wear

Please shower before coming to the centre and wear loose clothing that is comfortable and easy to remove. Please remove all make-up, nail polish and jewellery. If you wear contact lenses, they must be removed prior to surgery. Please bring your lens case with you.

What To Bring

Please bring all X-rays or relevant medical results, health fund details, medicare card, medications and medical history. Please do not bring valuables or jewellery.

Children

If your child is having surgery, bring along a favourite stuffed animal or toy that will comfort and put your child at ease. If your infant is having surgery bring two bottles - one with clear juice and the other with formula. Parents are requested to remain in the premises during their child's stay.
